it is hard to make "plans" for someone with labile blood sugar problems. An endocrinologist is your best bet, for sure. This can have to do with hormonal problems, thyroid, adrenal insufficiency, he may need a gastroenterologist if delayed stomach emptying is a factor in his disease.
In general, for your meal plans, choose the "wise" sources of carbohydrates, those which come from whole-grains and complex-carbohydrate cereals.... these "output" more slowly and contain a good "bang-for-the-buck" in terms of providing nutrients to his body.
