This may be reactive hypoglycemia. It is happening a little quicker for you than most, in the two-hour frame, where many experience it in the 3-4 hour frame. You "react" with a flood of insulin to the carb intake, bringing your sugar level low. Then your liver "sees" it and starts outputting glucose, and you go on "oscillating" for a few hours in this manner, until things calm down again.
You didn't say what kind of therapy, or if you have a diagnosis of diabetes....
Sometimes people who "cover" with insulin get these kinds of things, too. Like me, if I overshoot.
