Question Help! I'm at a total loss...

Even when I first met my fianée he had a thin, 5'10.5" light framed body type. However, over the past year he's lost at about 16lbs... At 114lbs he has a BMI of 16.1. My fiancée does not have an eating disorder. He eats healthy food and junk at all times of the day. When we first noticed his weight loss, I took him to his doctor and he went through a battery of tests...thyroid tests, ultrasounds to check his organs...pretty much every test out there except one for celiac disease.
I'd say that my fiancée's eating habits aren't regular, so much as grazing. On a typical day he eats whenever he has the slightest hunger pain. He doesn't like to over eat in one sitting. Eating huge dinners at my parents' house, or his parents' makes him bloated, sick, and gives him the runs...
Last winter we found out he was lactose intolerant, plus he was on a co-op term (for university) and he lived with his aunt. At his aunt's he was constantly eating meat and potatos (with little veggie), and he was sick daily.
After all the tests he went through, he had a clean bill of health...I have no idea what's going on. I'm very worried, anyone have any ideas what it could be?
