What the heck is wrong with me?

I'm desperate and not sure what to do anymore. I'm a 30 year old female with a burning, shooting pain in my stomach, that radiates to my right shoulder blade.

Last summer, I began having intense pains in my right shoulder blade. I went to my PCP, had bloodwork which had a high level of some liver related enzyme. I can't remember which one off hand. He sent me to have an ultrasound of my liver. It came back normal. The pain went away and I never thought anything twice. No stomach issues at this time.

In the last three months, the burning, shooting pain in my stomach started. After living on Tums for a few weeks, I finally made an appointment with my PCP. He put my on AciPhex, which helped and had me tested for H. Pylori. Negative. Since then I have had an ultrasound, HIDA w/CCK, CT scan with and without contrast, and a blood test. I don't have any gallstones, wall thickening, and the ejection rate in my HIDA was 98%. Every test I have had has come back normal. Every morning since I can remember, I have suffered from nausea. I'm overweight. My mother has Crohn's disease.

My boss thinks I'm some sort of hypochondriac and thinks I'm faking my issues (I sufffer from chronic plantar fasciitis, migraines, and PMDD) in order to miss work.

Can anyone help me?

Did they ever say anything about your liver enzymes - like why they are elevated. Mine are elevated slightly - my alt is 65 but they concluded it was fatty liver. Now from what I read fatty liver doesn't cause pain but people on this board claim it does. Some people experience nausea and pain. I have nausea and still haven't found any answers with all the doctors I have seen. I am now going to a liver specialist next month.

Your symptoms may be related to the fact that your HIDA scan score is unusually high. Right shoulder pain and nausea are common gallbladder symptoms.

There may be several mechanisms at work. First, the gallbladder is squeezing harder than it is supposed to, which increases the pressure against the walls of the ducts that carry the bile out of the gallbladder. Secondly, by releasing too much bile, the bile is acting as an irritant to the digestive system. Bile is strong stuff.

From what I have read in some cases with very high HIDA scans they wind up surgically removing the gallbladder. The next is speculation: there are studies that indicate that PPI's (the class of drugs that includes Nexium, etc) may weaken the contractions of the gallbladder. You will want to consult the best gastroenterologist (GI doctor) in your area.

Thank you for your help. I do have an appointment with my mom's GI on Monday. I'm nervous. But at least with the board and for those of you that are kind of enough to help us all out, I know I'm not alone.

To further complicate things, I've had zero issues with my stomach since Saturday. My husband thinks I should cancel my appointment, but I'm definitely not.
