Abdominal Discomfort - Where do I go from here?

Not sure where to go with this. I'm a 38 year old male and about 3 years ago, I started having lower right quadrant abdominal discomfort which would come and go throughout each day. Some days it was very mild, almost not noticeable, and other days it bordered on pain, but it never stuck around for more than an hour or two and never got above a 3 or 4 on a 10 point scale (1 being very mild, 10 being dibilitating).

I saw my GP who poked, prodded, did blood tests and concluded that it wasn't acute appendicitis so he sent me to the GI clinic. The GI doc did a thorough initial exam, poking and prodding a lot differently than the GP had and came to the same conclusion that the appendix wasn't likely involved. We then went through a barium swallow w/ x-rays of the upper GI tract, an abdominal & pelvic CT scan with contrast and a colonoscopy...all showing me clean and healthy as can be. He paid special attention to the appendix area and to where the small intestine empties into the colon to verify that no Crohn's was evident and that the appendix looked ok "in person". He concluded that my pain/discomfort must be musculoskeletal and sent me on my way

Last May (2009), I began also having upper right quadrant fullness, pain and discomfort, which ended up being attributed to a slightly fatty liver and and several gall stones. The gallbladders was removed in July with no apparent complications. I've felt pretty good since then, with occasional upper abdominal discomfort if I get carried away with what I eat, but I exercise almost daily and am making great progress in changing lifestyle permanently to a much healthier one than I had the past 10-15 years.

For the past 3 weeks, in an effort to drop the last 35 pounds to get to my ideal weight (I've already lost 60lbs but had plateaued), I joined Weight Watchers Online and have religiously stuck to the program. Last weekend, I noticed that annoying lower right quadrant sensation returning, but didn't really think much of it since I still get twinges off and on. This time, however, it has persisted...back to where it was when I had the full workup last time. Never painful, often slightly relieved by passing gas, radiates to the lower back on some days, not constant. This time, I now have intermittent discomfort a few times per day in my upper right & left abdomen as well as to the right of my navel and in my lower right abdomen/pelvic area. It's lasted for 4 days and hasn't gotten worse, doesn't wake me from sleep, etc.

I'm at a loss for what to do. I can't concentrate for long periods of time at work due to the discomfort and I'm sure anxiety about it isn't making things any better. Is it worth going back through the same thing I went through 2-3 years ago? If so, what do I do differently to have them check for other things? Could it be IBS? Kidney-related? Medication related (I'm on a low dose of BP medication and on Simcor for cholesterol control)?

I guess I should just go to the doctor again (he's gotten pretty used to seeing me with all this and my gallbladder over the past couple of years, lol), but wanted to see if this resonates with anyone on here?

I know your pain - I had mine removed 2 years ago but before I had it removed i was diagnosed with fatty liver. I get the slight pain under my right rib. I had every test you can think of and I was also tested for IBS which I don't have. Now I am seeing a liver specialist at John Hopkins - my symptoms are belching, nausea feeling from the indigestion, acid reflux, if I eat too much fruit the next morning my stomach will have spasms and I go to the bathroom several times. So something is not right in my digestive system. The specialist believes that I eat too fast, I eat too much fruit and I swallow too much air which all cause gas. I am getting tested for Sphinctor of the oddi dysfunction (he doesn't believe I have that but he is testing to make sure) and then I have to get a liver panel done to rule out hepatitis. He did say the liver will swell and cause pain when you have fatty liver. I have cut down on fruit which can make you extremely gassy and you want to eat small meals. I too get carried away sometimes when I eat and eat too much of a good thing. My pain is also like a 3 or 4 but never wakes me up at night. It never sends me to the hospital like sphinctor of oddi had done for most people so I think my problem could be more that I having delaying in digestion and fatty liver combined. All my anxiety has come back and fear the next pain. I would start researching and finding a specialist because your primary care physician will not have any answers for you. I found out that the hard way. I went to 7 different doctors and they couldn't help me so I took matters in my own hands.

Thanks for the reply, gigi. To be honest, I'm less concerned with the ongoing occasional URQ aches that I usually can tie to diet and more with the LRQ discomfort and fullness. Maybe I should be...

For kicks, I went ahead with my 40 minutes of high-intensity cardio exercise (on a stationary bike) a few minutes ago and it certainly didn't make it worse, so I'm even less inclined to consider appendicitis. I guess it could be referred pain/sensation from my liver or ascending colon. Just don't know. If it's not better by next week, I'm for sure seeing the doctor again, even if it's just for another referral to a specialist. Hopefully, it doesn't get worse. It hasn't really progressed in 4 days now, staying as a constant "full" sensation (maybe from me poking it all the time to see if I get rebound pain, lol) but with occasional twinges of discomfort around the right hip bone or just to the right of my navel. In fact today has been slightly better after a really good night's sleep (9 hours).

I have had the right side abdominal pain for 2 years, right above my appendix. The pain sometimes makes me double over in a sweat, can't eat, wakes me up. Been to ER several times but ct scans, blood work are normal. Sometimes it actually itches inside. I have had endoscopomy, colonoscopy, biopsy for celiac and all normal. My dr finally said after my last ER visit he would send me for a lapiscopic surgery. The surgeon didn't think I needed surgery but would assist an obgyn during surgery as I now have a new cyst on my ovary. (we know that isn't your problem) however, I asked the dr to remove my appendix at the same time as the hysterectomy I am going to have. I know it is rare, but chronic appendicitis does exist and that is where my pain is most of the time. Sometimes I will get it in my upper back but I have been told numerous times it isn't my gallbladder but maybe a kidney stone. I just started getting pain in my groin but dr said he didn't think my cyst would cause the pain. He thinks maybe a nerve from my back is irritating my front. In other words, I am not going to get answers until they open me up. Personally, I expect to find scarring on my appendix.

You said you have fatty liver - do you have a diet you follow so you can get rid of it? Do you feel pain with it? How long have you had it?
