What you have posted is the doctor's conclusion. SSA does not accept conclusions from a treating physician. This is because "disabled" is a legal concept defined by SSA, not by doctors. What is musch more important is the objective medical evidence, clinical findings, lab results, etc. that support the diagosis and the severity of the particular condition(s). So the letter itself may be helpful but only in conjunction with your ongoing labs, chart notes, etc. from your treating physician; especially as they speak to your limitations, physical and/or psychological.
