New DR, how do I tell him I'm need his help for SSD approval... - Disabilities Message Board

I don't know how to bring it up when I go to my appts. I have new Ins. and I've had to change Primary DR's 2xs, (they were terrible). I feel awkward asking them for their support w/SSD case. My records from prev. DR's haven't been transfered yet, last appt was on 6/20/08, DR. stated it takes a long time for recd's to transfer because they have new system.
Can anyone please let me know how I should bring it up.

Well, when I had to change cardiologists, I went to the new doctor I told her, that one of the reason I was with her, was b/c my last doctor did not pay attention to me and my issues with my high blood pressure, I also told her that I was pending SSDI and her support will be appreciated. she told me that once she gave a look over of my whole file, and was treating me for about 2 months, she would see what he could do. I have to be honest, it was almost half hearted. But it turns out it did not take that long, she keep adding blood pressure meds weekly till she was able to get my pressure in the High normal range. She filled out my adult RFC form, stating that all my meds made it impossible for me to drive, stay awake, and my blood pressure was "not stable."

Just be honest, If you can get your records from your last doctor and carry them in yourself. Most doctor offices will put your whole file on CD.

Sounds good, I will try asking him that way. I never had an RFC form filled out by my DR., only SSD forms were for me to fill out. Where can I get one? One of the questions my DR. asked was how high my pain level is, I said a 9-10, he then said I will put a 10 level. I hope this will help too. The bad part w/this new DR's is they don't accept walk-ins like my other ins. did.

Most lawyers have them and send them to your doctors to fill out. It is really just a bunch of questions that sum up your problems, and how your doctor feels it will effect your daily life.

Lynn...any idea how I can get a RFC form for my doctor. I don't have a lawyer right now. I'm appealing to the appeals council.
