Get copies of the doctor's notes which will include all the information he gathered from you personally as well as tests and results. It should list your height, weight, mood, a pain rating that you give the answer to, and what symptoms are obvious and if there has been any positive or negative change(s) as well as any changes in medicines and why.
I cannot stress enough to be very specific and if you feel something was dictated into your chart incorrectly, immediately bring it to your doctors attention. Make copies and have them notarized if possible. Send a copy to your advocate or lawyer if you are using one, send a copy to Social Security or have your advocate/lawyer do it, and finally add the original to your file of info. My lawyer is slack about updating SS, but we are so far behind our hearing that he is in no hurry. ARRRGH.
