LTD and two year mental health limitation

Is anyone here having this issue? Most LTD policies have a clause that limits payments for 2 years max. for mental/nervous disorders. I have a group policy with my LTD carrier that fortunately does not have this limitation. I say fortunate b/c I am no longer able to work due to a lifelong history of OCD, panic, and severe GAD. AS long as I remain disabled according to my carrier and my company's SPD, the max. benefit duration is to age 65. Can anyone tell me if the policy did have a M/N clause, where would that be found in your policy? Would the insurance company notify you of this once they approve you for benefits for a mental health disorder?

You would be notified in writing upon claim approval or at any time that m/n becomes the sole disabling condition (i.e. first out for cancer which resolves fully and then diagnosed with depression).

Where it is in the policy varies greatly as every insurance company structures their plan/policy documents differently. Often it's found in the area of the policy document that describes Benefits with bold titles such as "Limitation for Mental Disorder, Alcoholism and Drug Abuse" or "Mental Illness, Alcoholism and Drug Abuse Limitations" or "What Disabilities Have a Limited Pay Period Under Your Plan", or "Benefit Limitations" , etc.
