OIC, you mean you have not even applied for SSDI or SS retirement. What is your age? I know once you get close to retirement age, they start sending these things out sometimes. I started getting AARP stuff after my 48th birthday and you have to be 50 I think to qualify. Go figure.
I have received my Medicare 2011 Booklet, but, have never received Approval Letter and, certainly no back pay. Why would I be sent the booklet?
Maybe it was just for imformational purposes only. Until you actually have a medicare card you are not going to recieve any coverage. People who do finally win their claim for SSDI have to wait until 2 years later to get medicare. Most of the time people are eligible right away because their disability date of onset has been longer then 2 years. This is what happened to me.
Sounds like you are in the process of getting some type of disability though, but typically a booklet for medicare is not sent out until close to retirement age or close to winning your claim .

I applied in Jan. 2010 and verbally I was told I had been approved by someone I know in SSA. She said application was reviewed and change the onset date from Jan. 2009 to Jan. 2008. They said I should receive backpay by end of May 2011. but, after reading some posts saying they had to wait up to 9 mos. before they got paid. And, I had heard they will pay in installments. Is this correct?

Verbal confirmation is great but I would want it in writing. Have you received anything in writing yet?

social security will send a approaval letter to you. The letter will be detailed and tell you how you were approved and what for. it will have your date of onset, and when to expect your first check. And how much you will recieve. If you were approved for SSI then the backpay is paid in installments. SSDI would give you the backpay money right away.

The verbal approval actually came after having spoken with a past Lt. Gov. from Okla. that I knew and she told me she would get right on it. And, to expect a call from SSA. Within a week, I received that call saying I had been approved and should receive my check by the end of May 2010. I was mainly concerned with the "installment" payments versus a lump sum. Explain the birthday and how it determines when you are paid mscat? I don't understand this. Let's say my birthday is April 17th for example. Would I still be paid on the 1st of each month. My mother gets her SS retirement check the 3rd wed. of each month.

Well that one answer is on the social security website. I can't explain this to u . Typically people on SSI are paid on the 1rst. I have SSDI and are paid on the 3. People who are retired also have a different paydate. you will need to go to the offical Social Security Website to learn more about this.
About the backpay, what happened was about children recieving SSI in one lump sum. Social Security tightend that up, so mow parents not only are paid in installements they have to account where the money was spent and how. It is suppose to be spent directly for the child's needs. For adults, on SSI , I thinkit was just easier for them to be paid in instalments because they can only have a certain amount of money in the bank. If they go over they could lose their benifts. 2000 is the max they can have at one time in the bank. So paying ininstallments probably keeps them in line for the income restrictions SSI has. I believe they also have a certain amount of time to spend the money, especially if being paid installments. Because SSI is income based, and has very strict rules about this. It just makes it easier for the person on SSI not to go over the amount allocated, or lose their benifits, or installments. Let me go ahead and check the S.S website real fast on when amounts are paid.
There is a calander on the website , but could not paste or copy it. A lot of it depends on your birthday.

May 2010? If someone told me verbally I was approved last May I'd expect to have an approval letter within a few months. Quickly contact SS and ask for case status and tell them WHO told you that you were approved for what EXACT benefit and the EXACT date you were told this.
