Question Ssdi rfc forms.

Please, does anyone know how SSA decides to have you complete an RFC form? I applied in 3/2011 and have submitted all required medical records and other forms. Waiting for a decision now.

I did not get anything from SSDI asking me to complete RFC. Do they send it to you or to your doctor? Will the doctor tell me if he gets one?

Thanks in advance!!

Re: Ssdi rfc forms.

Sorry I should of read your thread more closey I see you filed in march I'm sure you wil get something soon. I remmber they sent you a letter saying you were award ssi but you had to go to a CE. How do you feel that went? I just looked it up your DR. probly has filled that form already where I looked it said no case can be closed with out it meaning you have to have it. So I bbet your DR. was sent one. It is something the DR fills out only.

My atty is the one who sent me the RFC to be filled out by my doctors. It is quite different than the one you can find on the internet, as the questions are custom to my disability.

Re: Ssdi rfc forms.

I have never recieved a RFC form Or one for my son. I am assuming that goes directly to your DR. in most circumstances. The only way to know would be to obtain your medical records. Or if you have a Lawyer . Just a guess.

Re: Ssdi rfc forms.

I didn't see or hear of an RFC form until I hired an attorney to file my appeal. The attorney gave me the forms to take to my doctor to fill out. Hope that helps

Re: Ssdi rfc forms.

I recall reading somewhere that SSA may not always request that a RFC (residual functional capacity) form be filled out but your SSDI lawyer or advocate may print one out from the SS website (or a slight form variation version) and have you take this to your doctor to complete to further demonstrate to SS your inability to do SGA. I don't recall my doctor doing one for SSDI but I do know my LTD carrier requires these periodically from my doctor to substantiate continuing disability.

Re: Ssdi rfc forms.

You guys will love this. I requested an RFC form from my local SSA. The lady actually told me she had never heard of this form lol. This only adds to the pile of screw ups they try and pull!

Re: Ssdi rfc forms.

On my application I listed: Chronic Lower Back Pain, but my doctors determined from my MRI results that I have: Cervical, Thoracic, Lumbar Degenerative Disc Disease, Degenerative Joint Disease, Advanced Arthritis, Facet Joint Disease. All with nerve roots impacts, such as pain. I have problems walking, sitting, standing for more than 30 mins.
My doctor has suggested surgery in the neck and lumbar. I am looking at a second opinion.
I have read a lot about people meeting listings and still denied. I will be 53 yrs in 04/2012I wonder if that makes difference.

I first applied in April 2011.
A month ago, my Disability Determination Specialist said she has received all the information she needs. Yesterday, when I called back, she said she has not had a chance to look at my case, because there are 15 other cases ahead of me, and all, like mine, have a lot of documents submitted. She did not tell me when she might get to it.
I wonder if she may ask me to fill out and RFC form after she reads my case or not.
Thanks for your help.

Re: Ssdi rfc forms.

if I remember right? my rfc forms were filled out by my physical and occupational therapists, while I was still in rehab, back in 1993 after being denied. my exam lasted 8 hours, 4 hours one day and 4 hours the next. it involved, comprehension/written tests as well as mechanical and physical abilities, also cognitive tests. they had me reading stories, then typing reports of what I'd read, they had me trying to put nuts on different size bolts and arranging them according to size. thet did not have me trying to lift things to carry so far, as I'm totally paralyzed from head to toes on my entire left side, they did not have me walking or standing to see how long I could do either, they had me doing math and measurements problems, as I'd worked construction all my working life. the math and measurements things I aced no problem. the cognitive, comprehension and mechanical things I failed misserably, as typing and screwing on nuts one handed took way too long!

It surprised me they allowed my son to be in the room with me during all tests, the therapists were behind glass doors watching and timing me? we were allowed to talk but he couldn't help me in any way. it was still May 1995 before I was approved shortly after leaving rehab, and my injuries happened on November 2nd. 1992. I don't know if any doctors ordered the rfc exams? if my lawyer did? or if SSDI ordered it done?

good luck and god bless

Larry/coupe

Re: Ssdi rfc forms.

I think you will hear something shortly . They have to make some kind of decison by six months at the latest since you applied In April 2011. Their is a 5 month wait time that no benefits are paid then after that social security starts paying benefits on the 6th month if their is an approval.
