CDR review Form-Can't deliver

Hi I just wanted to check in with all of you about my first review after 3 years on SSDI.
Just a reminder I got a notice that SSDI was going to do a review, they sent me a short form to fill out.
Well after leaving it on my kitchen table for a few days, I finally got brave and filled it out.I decided to send it "certified ". Well that was a mistake, according to the post office, they tried several times to deliver, but said no one qualified was ever there, to accept it.
So they left them a note, this was last wedesday. so I am really stressing now. I am going to try and call, and let them know that the CDR is sitting in the post office waiting for them to pick up.
I have visions of being cut off, all because of this, i want to scream.....
Well word of advice, Don't send certified.......Thanks for letting me vent, I will keep you all posted.

Re: CDR review Form-Can't deliver

I am so sorry that happned to you How long did social security give you to fill the paperwork out and mail back? I would call my local offica or go down there and get anther CDR form . Re-fill it out and send back . i don't want the time to lapse because they did not pick it up at the post office. Some of the forms are computor generated and all they do is run it back into the computor, i do not know who looks at the CDR's, I have not had one yet. It looks like many people are getting them.

Re: CDR review Form-Can't deliver

I wish they would let you fill all this stuff out online and submit it. I usually send everything certified too. I find it hard to believe that no one was there to sign for it. I have never had this problem. Questions? Where was this letter mailed? Was it to your local SSA field office? Also, was it addressed to a specific SSA employee? Definitely do as Mscat said and either call or go down to the local to explain the situation. I think this is why it is important to fill out the forms properly and submit them back AQAP. It gives you some leadtime on the deadline if there is ever a problem with delivery. If this stuff goes to the local, it might not be a bad idea to just hand deliver things yourself.

Re: CDR review Form-Can't deliver

Since it was sent certified, it isn't lost in the mail and you could request from the USPS a return to sender and call the SSA and also request an extension on the deadline after giving them an explanation. Although the easiest thing to do might be as Mscat suggested....just fill out another short form and resubmit altho this might not even make sense if you have to mail the form again.

How long will the post office hold onto it before sending it back to you if no one picks it up. Will the post office attempt anymore deliveries or must someone from the SSA go down to the post office and pick it up now? I know UPS will attempt about 3 times before leaving a final notice, altho I would never use UPS for something like this.

Re: CDR review Form-Can't deliver

Always make copies of every form or piece of information you send to SS or Medicare or LTD or any important piece of mail. Then you have a copy to send if the original is lost. Unfortunately most mail today for things like SS go to processing centers where SS may not be running the operation - many companies and agencies outsource to advanced companies for mail receipt. I once worked with a company doing this for a major insurance company I was working with.

I just filled out a survey on my hospital stay and wanted to add my own comments but I saw the form going to another state and the hospital is wholly owned here. And the form said "don't include anything but this survey as it will not be retained". I kept the copy of the survey as I do hope to talk to the hospital about my areas of concern.

Even when you want to contact your magazine company about a magazine - you aren't talking to them. You are talking to a processing center that does the work for the magazine company. The person may have just spoken to a Time subscriber and is now answering your call regarding Redbook magazine and next will talk to someone about Rolling Stone.

Re: CDR review Form-Can't deliver

Good advice Spine. I have electronic copies of everything I have ever sent to SS and my LTD carrier. I can always pull up the copied images and print out hard copies for re-mail or fax the copy images.

Re: CDR review Form-Can't deliver

Will the post office send back the paperwork to you? Maybe you might want to go to the post office and see how long it takes for that to happen? Then Make copies of the CDR, and mial again ? It is weird that it did not go through.

hi all thanks for the helpful hints. I found out from the post office that a recent update that its on its way back to me. The problem is, it did go to a contracted processing place, contracted by SSDI. So this place has no physcial person avaliabe who can sign for certified letters. It went to PA.

I called local office, they made note of the problem and told me to bring it in to them, they would forward it. To answer someones question, i had 30 days to complete.

So certified, not always a good idea. but I do need to make a copy of it, thanks for all your suggestions, i will let you know how the review comes out.

Re: CDR review Form-Can't deliver

Question? Had you not sent it certified, where would the post have delivered the mail to? Was this like some unattended drop box where someone comes by daily for pick-up? I suppose SpineAZ gave the best advice. Just make a copy and send it regular mail. Here is what stepmum wrote in an earlier thread
Quote:
The short form (SSA-455-OCR-SM) is the “easy” one. It is only two pages long and is read by computer (OCR stands for Optical Character Reader). Since it is sent primarily to beneficiaries who have a low probability of medical improvement, this is probably the form you will receive.

This is good in that the short form assumes you are still disabled unless you give them reason to question that. A human only looks at the short form if the computer kicks it out for one reason or another. If it isn’t sent to a human for further review, then your benefits continue uninterrupted.
My only concern is that the resolution from copies is usually less than the originals esp. if you are using older hardware technology (scanner, printer) to produce the copy. If you send a copy, I hope this does not cause the OCR to kick it out and thus has to be examined by a human which could trigger a long form to be sent.
Quote:
The long form (SSA-454-BK) is 10 pages and is very similar to the original forms that the individual filled out when he or she first applied for disability benefits. The form primarily asks for the names and addresses of all medical providers. SSA will obtain the medical records directly from the medical providers. This form not only goes to persons whose condition is expected to improve, it is also sent to persons whose short form was “kicked” by the computer, and is randomly sent to persons as part of a trial or study that the Social Security Administration may be conducting.

Re: CDR review Form-Can't deliver

Yes I did send it certified. I normally would reccomend this when written communication is done with any social Security or any other goverment office. In my case with this first review and it was a short form with little dots to fill in or squares. It does go through some processor machine, and knows when there is a red flag, and kicks it out(so i have been informed). If it gets kicked out a human will review it, then process it.(like stated by blueSkies).

So I was told by Post Office,If it was sent regular mail, it would be deliverable. Apparently there was no one to sign for the letter, I do find this strange, but i remember all the hell I have been through with these people in my 6 year fight for SSDI.

So now I wait and see,I will let all of you know what the out come is thanks.

Re: CDR review Form-Can't deliver

Thanks stepmum. I am sorry about all the anxiety that this has caused you. This is a very valuable thread however. I always try to learn from the mistakes and experiences of others. Now when my 3 year CDR comes around, I will know exactly what to do if I receive this short form. I will also probably send the long form via regular mail if I get that after making copies of everything.

Perhaps 3 years from now, you will be able to just login to your account and fill / submit this stuff online.

Yeah, I did not go thru a 6 year battle with SS like you stepmum but we all can empathize with your fears and concerns about possibly being kicked off after fighting so hard to get approved. Everyone will have this same anxiety when they get their CDR. My only hope is that SS will make things easier for those who are truly disabled and harder for those who are not.

Question? When you get this letter back from SS and make a copy, are you still going to take it to the local and let them forward it or just send it back regular mail for 44 cents? Curious.....How much did the certified letter cost you?

I recieved it today, took it to local office. They marked it as recieved, they will forward it to PA, the processing center. A certified letter cost 1.40, not bad, worth it in most cases. So know I sit back and wait .
Thanks for your advice, I appreciate it.

I have also learned a lot from this thread, thank you for posting , and sharing your experience , stepmum. I have not had a CDR yet, and I was approved in 07. I think a CDR makes most of us very nervous , but I have heard most people are found to be disabled after the review, Do you know that once you get the letter , does social security give you anther CDR date?
