Dr.'s sign says no form filled out here. What to do?

So at my psych office they have a sign in the lobby that states they do not fill out SSDI, Disability forms, FMLA, and that we should go back to our original referring Dr. Well my problem is that I was originally sent there by their mental hospital because I only have insurance through the state. My question is if I'm only able to go to them for my meds and appointments does this mean SSDI will only be able to go off their notes if I can't have them fill out a SS form? Thank you guys for any help. I'm at reconsideration phase.

Yes, if you go there for treatment, the doctor will have to take office notes and be required to submit any requests for these. Unfortunately, I see this more and more from psychiatrists. They are eager to talk to you for 5-10 minutes, take your money, medicate you to death with addictive meds (some of them), but will not help you fill out any kind of disability paperwork. This is so unfair to the disabled patients IMO. Screw these guys...I'd start looking for me another doctor. I mean the only way you are able to pay them is from the disability benefits you have. If they refuse to help you and you lose benefits, you can no longer afford to see them for treatment. It is really crazy yet if these same doctors became disabled themselves, they would want another doctor to help them. These doctors make me really angry. They are only interested in making money for themselves. They see filling out any paperwork for the patient as a tremendous chore and burden.

Many docs won't fill out the massive amount of paperwork for SSDI or that is all they'd do. So ask your doc the next time you are in who can do it. They often have an offsite place that does it....for a fee. Or just call and ask for medical records and ask how you gt the paperwork done for SSDI or do you?

In many cases, they want you to use the SSDI docs to evaluate you. And quite frankly, from what I've seen, your own doc who knows you is often overridden by the SSDI docs anyhow so they feel it is not worth their time to do the paperwork if this one time evaluation is the one accepted and not their in-depth evaluation of you over months or years.

Just ask and don't accept a sign....they put that out to discourage the people who "shop" for docs who will give a good report......and yes, people do shop to find a good report. But I bet they will tell you to do the eval with the SSDI shrink.

At my spine specialist's office, there is a sign saying that they charge for filling out disability forms. The sign lists the common forms, and how much they charge for each one - just like a restaurant menu. Being a spine specialist, I bet he fills out LOTS of forms.

When I told my attorney about the sign with the charges listed, she said, "they can't do that!". I don't know how she did it, but she got everything she needed from the doctor, without paying a penny.

Although my attorney never said it, I definitely got the impression that many doctors don't want to fill out forms, and will try just about anything to get out of doing it.

If you have an attorney, they will know what to do.

You are right Pete. Most patients are willing to pay the doctor for the time it takes him to fill out the paperwork but some docs will just flat out refuse to do it for a fee. My psychiatrist told me point blank that he thought I was unable to continue working yet also told me he quit filling out SSDI and LTD forms for patients b/c it got too overwhelming and his primary focus was to see patients and not spend half his day filling out forms. As it turned out, the forms really are not that bad. The doctor could even take a few home daily and fill them out in his leisure and charge you if he wanted. Many psychiatrists are lazy. All they wanna do is prescribe you a cocktail of drugs and keep you coming back for refills. Many have no incentive to see their patients recover.

thanks. I'll pay for the forms but it just says they won't fill them out. I'm going Monday and I'll let you guys know what they said. My attorney said just to ask politely

I thought Dr's can now fill out these forms electronically ? Social Security made it easy for Dr's all they have to do is read it and sign, then it goes in your records for S.S to review or your Attorney. Maybe your Attoney could influence them to do it.

Yes, since you have an attorney, I would get them to try to obtain the forms from your doctor. That's what you are paying them for. If the doctors office won't fill them out for you, chances are they will for an attorney. Good luck, and keep us posted!!

Well just update. Spoke with the Phych ward and they gave me all my records for my attorney. I asked about the policy of not filling out forms for ssdi and she said that was correct incase people "shop" for it, but ensured me that on ssdi cases they state their findIngs and said they are mostly accurate on situations like mine to where I would be found disabled. Whatever that means I'll ask my attorney. So I guess that's ok for now.

I know this sounds weird to say but I'm so glad they are sending those records in your behalf. But at the same time it means your (disabled) it hit all of us hard when DR. tell you to file are you can't work. I mean I'm happy they are supporting you but sad for you to. Also did they give the copies that kind of unuseal in an mental case. Hope your have a better afternoon. God Bless you
