I was wondering who you go through to get a Disabled Card to put on your rearview mirror. Do you have to go through the DMV or through SSA to get one and is there a application processe. Can you get one automatically if your Disabled or do they only give them for certain disabilities..thanks

Super easy really your doctor fills out a form and you take it to the DMV you get the placard, tags, or combo of both. My hubby got both as we sometimes go places with a couple of our children. The placards are free in our state to change the metal plates was a $5.00 one time charge. I do not think all states charge to replace the metal plate.
Call your local DMV office and ask them - I think each one has different rules.
I called mine and they told me to have my doctor write on a prescription pad that I needed a parking tag. Strange, I know, but that is what they wanted.
My doctor's assistant wrote it out on the prescription slip - I took it to the DMV office - and got my tag.

I went to my states DMV website & printed off the application. Took it to my Dr. to sign & then mailed it to the address on the form. Got plackard back in about a week. It was free this way.
They have temp. ones or perm. ones.

I got the form off of the Arizona DOT website, took it to my doctor with my portion completed, he signed and I took it to the DMV that day and got my placard.
In Virginia you have to pick up a form for the Dr. to fill out and then return. It cost 5.00.
You don't necessarily have to be "disabled" to get one. I got one because I had chronic plantar fasciitis and tarsal tunnel and it was difficult for me to walk.
I wished some of the other responses had included their states, I wonder which ones the placards are free. I used to live in NC and they were 5.00 too.

For California , i went into the DMV office and the woman at the counter handed me a form and told me to have it filled out by my DR. So I did, brought it back and was given my placard , and also got my handicapp plates for free, i had someone help me remove the plate for me , they were all so nice about it. SSA has nothing to do with the PLacard's
Florida: Must follow guidelines under Florida statutes. It apparently used to be pretty liberal and they've clamped down on these rules over the last couple years. I have mine and had no problem getting it. Doctor just has to sign the form..
Form can be found online or doctors offices have them.
If its a temporary permit $15 = 6 months
If its permanent FREE.
Both of the above are the ones that are assigned to your license tag and hang on your mirror.
I don't know how the metal plates are handled. I haven't looked into it - yet.
