They can't figure out why I am having trouble swallowing:(

Here is my situation
I am a 29 year old male. I had a sudden onset of dysphagia at the end of April.
I have been to the ENT, the GI doctor, and they have done all of the tests. They cannot figure out why I am having trouble swallowing..
Here are the tests I have had done
1. Regular and Modified Barium Swallow-nothing came up
2. CT Scan
3. X Ray
4. I had a ENT do a scope of my larynx/throat. Nothing came up. He put me on prilosec since they thought it may be acid reflux. He was not sure
5. I had a GI doc do an upper GI-nothing came up. They did a biopsy, and found I had some cells in my esophagus (Eosinphillic) but it may be due to acid reflux or allergies. I had been on Prilosec. I also was put on sigulair. They insist that due to the number of cells (very few) it could not be the cause of my problem swallowing. I had less than 5. You need usually more than 20 have dysphagia.

It feels like food gets stuck in my throat and I need to cough. Today it is worse. I actually have a sore throat right around my adams apple.. I am afraid to eat solids when I am alone now. This couldn't have come at a worse time. I am scheduled to go back to grad school this fall, but now I am considering not going..I can't go and be busy and worry about eating..I wish I could be normal again. They are thinking this is in my head..and some of it may be, but it defiinitely has a physical component..I was fine until then..this has gotten very strange and I am ready for a nervous breakdown.I wish I could be better

Your Physicians sound incompetent. You should change your Physicians for ones that are more intelligent and better educated.
The symptoms you describe is that of a condition(rare) call Achalasia. They (Physicians) can confirm the Dx by doing an Upper Endoscopy.

I agree with the upper endoscopy, they need to find out if you have motility in your esophagus. Your esophagus has 3 contractions to push the food down, if there is no motility you will have a hard time swallowing. I have achalasia and they did a dialation of my esophagus to help the swallowing problem, they cannot fix the motility problem so I will always have some sort of problem in the esophagus. I just try to eat soft foods and absolutely no meat, nothing real solid. Sometimes even bread will cause a problem.

Good luck and whatever you do, go back to school. Let us know how things are.

How did you get diagnosed? Did yours come on all of a sudden? I seemed to have sudden onset of my dysphagia..it came on the end of april..
I had two barium swallows and nothing came up...from what I read, there should be an anomaly on these tests.. I thought the doctors I am going to were among the best, at the University of Pittsburgh Medical Center...But I should look at the doctor individually instead of judging them by their institution. The doc told me on Friday she thinks its all in my head since they cannot find anything wrong with me via the testing...

I am honestly about ready for a nervous breakdown. No one can seem to find the cause of my problem. I now have panic attacks when I eat solids..but its because I have been having so much trouble for the last month and a half..I didn't have these at first so it can't be all in my head.. I have an appointment with a shrink on Wednesday..Maybe if I get committed they will run some more tests on me and find out what is going on down there.

Hi, mine did not come on all of a sudden, but slowly. I went to doctors for 2 years and they kept telling me I was constipated and gave me stuff for that, and I was not constipated. I finally got referred to UCLA and when I went I could not even swallow a teaspoon of water. The first thing he did was the endoscopy to check and see if I had any motility. I did not and never will. They did the dialation which probably helped, but it also ruptured my esophagus, it was as stiff as a board. There is only a 5% chance of this happening.....my luck! I could have had the surgery instead which would have kept me in the hospital for 2 weeks, so I opted for the dialation. It kept me in the hospital for 17 days and I almost died. I don't want to scare you because this happening is very, very rare. Most people get these dialations with no problems and have them periodically.

So find yourself a very good gastroenterologist and ask his to check your motility, and don't eat solids or anything that can effect your swallowing, like breads that turn into little dough balls and have a hard time going down. I am sorry you are going thru this, I really don't believe any of it is in your head, when a doctor can't figure out what is wrong, they automatically go for "it is in your head", don't let them get away with that. Keep searching and don't give up, as I said, it took me 2 years, it shouldn't take that long. Let us know how your are doing.

I called the GI doc t oday and spoke with the nurse. She then spoke with the doctor..who basically said for me to consult with a therapist..they can't find anything wrong with my swallowing. I tried to explain but the nurse had to go.
I am pretty upset about this. I am getting my records tomorrow and sending them to another institution and making some phone calls.
I can't believe this

Panthers,

When they did the test, did they actually put the scope down into your esophagus? And down into your throat, or did they just do an upper GI? Your Doc really does sound incompetent, I am glad you are going to change. Please don't give up....

I had two scopes
The ENT doctor did a scope of my upper throat to make sure everything was closing up and was protected when I was swallowing. He is the one that suspected acid reflux. I have been on a PPI since May 7th..

I went for my second opinion to a GI doc..the Doc that did the scope was highly respected ( I checked him out) but he was not the doc treating me. They just used him to do the procedure. He went the whole ways down to the entrance of my small intestine..
The only thing he found was that my esophagus has mild felinization which means there were some "lines and furrows" but those by themselves do not cause problems. He did two biopsies of my esophagus when he was there..I had some of those eosinphillic cells which can cause esophagitis, but I had very few of them so they can't consider it to be allergic esophagitis. More than 15 are needed per sample, but mine had less than 5 so my doc thinks its reflux..

Tonight I had trouble with rice..anything really hard gives me trouble. I have no problem whatsoever with soft stuff I think I am going to need some time off of work. I was going to quit next month anyways before I go back to school..

Hi,

I read your posting and thought of some things that might help. Are you taking any medications. There are some medications that cause dysphagia. Are you experienceing any other symptoms or anything else out of the ordinary. In addition, do you have trouble swallowing intermittently or consistently. Is it possible that the dysphagia did not show up on the MBA (modified barium swallow study) due to intermittent dysphagia (asymptomatic during the actual study). If it is intermittent, do you notice any patterns such as time of day or when fatigued? What are your symptoms: i.e., is there coughing/choking during the swallow?, is there the feeling of food getting stuck? and if so where does it feel "stuck". I know you mentioned that you already had one barium swallowing study, a modified barium swallowing study, GI series and endoscopy. Did you have a manomatry study (i know I spelled that wrong); basically it tests the pressure and muscle movements in the esophagus. Just some things to think about before persuing it as psychological. Hope it works out with your new doc. Good Luck.

This is James. I have two accounts.
It can happen any time of day...no set pattern
It feels like it gets stuck around the Adam's apple area.
I am not sure if I am just not swallowing right due to anxiety? I am getting really paranoid now

The only medications I am on has been prilosec and I talked my allergist into giving me singulair in case it is allergic esophagitis
Other than that, no medication

I have an appointment with a speech pathologist next Friday to go over my swallowing, and a followup with the ENT the following Wednesday

I am trying to find the name of a good swallowing doctor in the Pittsburgh

I am just kind of caught in the middle between GI and ENT since I think the problem is right in my throat..I am not sure what kind of doc I should be going to..I am not sure if the problem is at the beginning of my swallow and that is causing the problems?

I can't believe I found anyone else who has similar problems. My problems started suddenly - 3 yrs ago. ENT said it was Tonsilitis. I felt like something was stuck in my throat and had phlegm constantly in my throat. I was on antibiotics but it NEVER went away. I've had tonsil stones for quite a while and he said my tonsils were constantly swollen, that could be the cause of my problems - or maybe GERD. Saw a surgeon - had all the tests done for acid reflux (which I had had for years) and they came back that I was having significant acid reflux, even though I felt like it was under control because I had been taking Prevacid for years. Since I was having the swollen tonsils, bad breath, food getting stuck, etc., I chose to have my tonsils out. I did that in August of 2006. It got rid of the symptoms I just mentioned, but not the others. It also created new problems. I've had to change my diet because I can no longer eat a lot of foods for various reasons. I have trouble swollowing a lot of foods such as meat, bread, etc. and they feel very dry if I eat them. If I do eat them I feel as if I'm going to choke. Ice cream - which I used to love - now seems too phlegmy or something, as does milk, so I've cut those out of my diet also. I've taken my Thyroid pills and then several minutes later coughed them up - because they lodge somewhere in my throat instead of going down. I constantly cough and clear my throat because of the phlegm and feeling that somethings stuck in it. My vocal cords are always irritated and I have problems raising my voice.
I've finally quit losing weight, but have gotten SO DISCOURAGED because through all of this the ENT told me there was no reason for my tonsillectomy to have caused these additional problems and this was just a "globus sensation" I needed to learn to live with (actually 3 ENT's). There is nothing wrong with me (althouth I've lost 70 lbs in the last 3 1/2 yrs and 30 lbs of that was after the tonsillectomy). My GP has been wonderful (actually she's a PE), she just hasn't been able to find a "Specialist" who can figure out what's wrong. In April of this year I went through all the horrible tests for the acid reflux again so I could have the Nissan procedure done to repair the Hiatal Hernia. I'm 45 and actually didn't think the Tonsillectomy was as bad as everyone else did. The ENT had me prepared for far worse and I live by myself and was just fine taking care of myself. I only took a week off work and had my 2 yr old grandson for the weekend 1 wk after the surgery (although he is very good). The worst part was not being able to eat or drink for so long. My surgery for the Hernia was much worse - only because of the gas they fill your abdomen with. I was in pain from that for almost a week and it affected my bladder also. This surgery DID NOT help any of my symptoms either. I still have a hard time swallowing the same foods. I eat mostly salads, which now give me a bloated, gassy feeling. It seems like anything I eat makes me feel bloated. I don't even drink as much liquids as I used to. Since my tonsillectomy I can't gulp very fast either. I've quit losing weight but I've gone through 2 surgeries, racked up $$$$ of medical bills I'll be paying off for the next two years and I'm still having the same problems. Went to a "Swallow Therapist" who said it wasn't muscle related so she couldn't do anything. She thought it was environmental or allergy. I've taken allergy meds and it doesn't help at all. I've had 3 sinus surgeries in the past and still do the nasal irrigations periodically - NONE OF THAT HELPS EITHER. Everyone just guesses. I think I had the motility test along with the PH test for the Nissan and about 1/3 of my swallows are irregular (the way my esophogeal muscles work) but neither the ENT or surgeon referred me anywhere or seemed to think that could be causing these problems. My GP suggested the dialation, but when I asked the surgeon about it, he said it was a waste of time because it didn't do anything. I'm starting to feel as if I'm wasting money on Dr's who are more concerned with their ego's than their patients and I'm about to go crazy from these symptoms that are "psychosomatic". Any suggestions????

Hi ~ regarding your post on healthboards...did you find a solution for your swallowing issue? Both my brother and mother have gone thru this. My brother they told for 2 yrs to go to a therapist (later to find out that his throat constricts out of no where and it is hereditary..he has to have his throat stretched about once a year) and my mother has to have hers stretched at least once a year. She skipped this past year and this past July her throat closed suddenly and she couldnt swallow. Hers goes back to when she was little and drank the poison lye and shrank her throat, but my brothers..they don't really know why it happens.

wow, this is horrible isn't it?
I have trouble swallowing too. I was diagnosed with lpr (reflux) but my problem is only a little bit better after more than 3 months on a double dose of prevacid. I have had almost all the tests done (endoscopy, laryngoscopy, barium, modified barium) but the only thing they could find was lpr. I also have a cough, globus sensation, tickle in throat, breathing difficulties and mucus in my throat.
This is totally destroying my life and doctors dont seem to have a clue about this.
