Constant Flem & Bumpy Roof of Mouth

For a long while now, I've noticed the roof of my mouth - near the back, where it gets soft - has at least a dozen small bumps on it, similar to a rash. It isn't sore to the touch, but I've always tended to get an itchy throat at least once a day and have learned to scratch it, albeit with an annoying sound.

Also, which I think is related, when I take a hot shower I can discard what seems like an unlimited amount of flem. It's mainly clear, sometimes a tiny spec of yellow.

What could this be? An infection? I've had this for a very long time, as I started scratching my throat almost 10 years ago. What do the rash-like bumps mean?
