I guessed that tk1's problem might involve depth perception because I have exactly the same issue. In my case, the source of my problem is a slightly damaged retina (secondary to an epiretinal membrane), which impairs my depth perception. I also cling to the rail when going down stairs, although I never had this problem when my retina was intact. (Unfortunately, I live on the second floor of a vintage building and regularly use the "el"--i.e., elevated train--for getting around the city, so I'm always using stairs.) Like tk1, going up stairs is not a major problem for me. (Difficulty going down stairs was actually a screening question at a vitreoretinal clinic I went to.)
