"Sick Journal"

Can anyone help me with information on making a sick journal? I recently applied for SSI because my fibro is so extreme is has consumed my life. I was told that I need to go back the last few months and start there but I cant remember exactly what I did each day or what meds I took that day. What types of things should I include. I was thinking to list what meds I took, what times and how much; what else should I be putting into this?

I keep a spiral book, it is smaller about 5X7 something like that. Anyway I use it to jot down pain level that day and anything I want to remember like extra meds or nausea or even things happening in my life that may cause extra stress etc. I also write down what I eat to keep track of the good and bad stuff I eat for my fibro. You have to take a few minutes during the day to write in it, but believe me I have them going years back and they have come in very handy when I needed to know about a particular med and how I reacted to it and when I started or stopped it.
