I have PTTD in both feet and have been in pretty severe pain 24 hrs a day from it. Cannot walk hardly at all and am working from a wheelchair at work. I have done the orthotic, PTTD aircast brace route and now am on my 3rd adjustment in an Arizona Brace. My ortho is an ankle specialist and has not yet talked to me about surgery. However I seem to have exhausted..oh yeh was in gortex permanent casts too.....all avenues. He wanted me to try the Arizona brace on one leg to see if I could handle wearing it. I have really tried hard...have had 2 adjustments and still is very painful to wear it as it is putting pressure on a tender point on the outside and upper part of ankle. So even though it is not as painful to stand, and only can do that for a minute or 2..on the inside part the brace is making the other side hurt now too. I see the Dr again in 2 days and had hoped to avoid surgery but I suspect that is my last option. He has not even mentioned possible surgery. I wondered if a Richie Brace would be more tolerable pain wise??? My fear is loss of my job due to being out on surgery...hence loss of health insurance. Then I would not have any funds get surgery on the other foot later. I also do not have one good foot to stand on while one heals from surgery? Right now I work in a wheelchair and that has been fine. I might be able to get out of work for a couple weeks and then work in weekchair from there on till recovery done. I was very active until this suddenly happened in Oct 2008 on both feet after over kill in physical therapy. And no I am not suing my PT because I cant walk now. They would not have known my tendons were so sensitive,e ct Has anyone had trouble with Arizona Brace also. Is a few weeks out of work and then back in a wheelchair realistic if I have surgery????
Wow, what a hot mess! I think what you need to do is get a second opinion and fast. My surgeon took one look at my foot and said surgery. I fought him on it, but only for a few weeks. I can't believe you've gone through so much and your doctor has never mentioned surgery. Why? It seems to me that your best interests are not his priority. Find an orthopedic surgeon who knows your condition and can make a professional diagnosis. What tests has your doctor done? MRI? X-rays? Anything that would indicate surgery as your best option?
Now, to answer your question - It is possible to have the surgery and be back at work after a few weeks, but it would be tough.

I agree. Four months conservative treatment was my surgeon's protocol. Insurance does usually require a period of conservative treatment unless it is an emergency situation, but yours has gone on way too long.
Well, I went back to ortho surgeon, who by the way is ankle specialist and here is the surprising outcome: He said he has never seen a PTTD case where someone has spontaneous tendon tears in both ankles and severity in only 48 hours after injurous exercise I did. The MRI never showed a tear and my arches, even though cant walk without huge amt of brace support are not collapsed. He does not think the braces are helping afer all since my pain never really changed in either the Arizona BRaced or the other aircast PTTD ballon braced foot. He want me to ween off the braces and try and get back into my orthotics with exercises and pool walking. Wow did not expect this. In other words he does nto know what is the problem. SO I started wearing only the aircast PTTD brace on both yesterday , removed the Arizona on the right and lowered the pressure ever so slightly in both balloons. I am bringing my orthotics to work today and might try them for a little bit. I will still use my wheelchair and the walker though. Prayers please that I dont blow both my arches out again also! I have been out of my leg ankle braces for 6 days now and doing pretty well. Still have ankle pain but is not worse and getting stonger. I attempted to walk across the mall where I worked. It was alot of work but I made it and then returned to the optical dept and sat and worked from the wheelchair. I started pool therapy last week . They are trying to strengthen the muscles in hips back and all the way down to my ankles and allow me to bear weight on those arches in the pool. Every day I try and walk a little more.
I had doctors putting me orthotics, braces etc for years saying oh..it is becuase you are fat. Well I am not that fat (high end of overwight BMI but not even obese) and letting it go caused me to reach PTTD stage 4 which when finally fixed required a triple arthrodesis fusing my foot. oprthopedic surgeon said it was crazy to let it get so bad. Could have been fixed more easily and better earlier.
Get an opinion (or 2 or 3) from good orthopedic surgeons who specialize in foot and ankle.
