Last week I had a cortisone injection on the top of my foot near the 2nd and 3rd toes. The Podiatrist sprayed something cold on the site of the injection as a distraction tool (I'm not sure if this numbed the area) and at the same time injected the cortisone. He told me to hold my breath, then he told me to let my breath out. The injection hurt a lot, but only for less than a minute and then the pain of the injection was gone. I've had no complications from the injection. The Podiatrist didn't tell me whether the cortisone injection was a long-acting or short-acting injection, though.
I cannot comment about how a cortisone injection would feel in a big toe, however, but I just wanted to provide you with some insight as to my first experience with a cortisone injection in my foot. I was extremely nervous about having the injection because my foot is sensitive and also because I hate needles. But the injection was well worth it...my toe pain/tingling has improved.

I am also going to physical therapy and having ultrasound treatments there.
