Well, what was the surgery you had? I had a cheilectomy 3 years ago on the left great toe. This was caused by several factors - the heels being one. That being said, the pointy toes of today are radically different fromt he ones we wore int he 60', 70's, etc. Those were closer fitting with the point angling in from behind the toes. The ones today have a wider toe box - for the most part - and the point is further out front of the toes. The bigger issue is the height of the heel. I really can't wear anything higher than 2 inches. It's the angle that it puts the toe in that causes the problem for me.
And I've found that those cushioned inner soles are a great help for thinner soled shoes. I dance and my dancing shoes are thinner, leather soled and this has made it much more comformtable. Hope this helps.
