I'm 23 and not on any medication now, nor was I previously. I haven't had any major diet changes in the past year, though I have recently started drinking a little tea (unsweetened) daily when I used to only drink water. The only exercise I do is walking and an occasional bike ride - no changes there either.
I might should mention that I've always had the tendency to eat large amounts of food and drink a lot of water. Sometimes people are surprised at how much I eat at one sitting considering how thin I am. Four years ago I ate mostly junk, and while I still eat large helpings, I have at least been eating much healthier foods the last four years. Luckily I've always had a fast metabolism and my body doesn't store fat very well. I might interpret this recent weight gain as the first signs of my metabolism slowing down with age if it weren't for the fact that I can't find any fat showing up on my body. In fact, when I pinch the skin around my stomach, it seems tighter than it did a few months ago.

I'd be reluctant to say it's impossible, but I would say it's highly improbable. I've been using for years now the same scale that I used today to further verify the doc's reading which nearly matched. I've also always weighed in at around 150 at doctor checkups for the past four or more years now.
