Cerebrovascular disease = conditions that affect brain artery health - Fatty deposits that lead to obstructive stroke, or aneurysms which rupture and cause death.
Chronic obstructive pulmonary disease = emphysema/chronic bronchitis. In both diseases, there is chronic obstruction of the flow of air through the airways and out of the lungs, and the obstruction generally is permanent and progressive over time. Asthma is not included in the COPD category, because the lung obstruction from it is reversible and not permanent - that is, between attacks of asthma, lung function returns to normal.
