Some people choose to go outside of their medical insurance to get treatment. I would NEVER encourage anyone to cancel medical insurance - you never know when you may become seriously ill and may need the coverage. But I know many people with health insurance who choose to pay on their own because they know the provider or the treatment won't be approved by the insurance company. It's not insurance fraud not to use it - but it is insurance fraud to fail to disclose prior treatment when asked.
For example, if you apply for Life Insurance and they ask "have you seen a doctor in the last year for cardiac issues?" you can't lie and say "No" in order to hide the fact you did see a cardiologist but paid the fees without filing for insurance coverage of that treatment. You must answer truthfully to any and all questions asked by an insurance company when seeking any coverage (be it medical, life, disability, etc).
