Are you talking about being a home health aide through an agency ? Licensing requirements are different from state to state. If you're going to work for a home health agency that gets Medicare and Medicaid reimbursement then you would have to be a certified home health aide in the state you reside in.
If you're going to be hired privately by your family member then you don't need certification unless your family member is being covered by an insurance company that requires certification or licensing in order for the patient to file a claim with the insurance company.
