22 year old male need some advice please

Hello,

The short version is May 6th I passed out randomly, ever since then from the second I wake to the second I go to sleep all day every day i feel "buzzed" or like i am in a dream. I have had alot of heart tests done with all good results...when i wore a holter monitor they found 14 hours of bradycardia with lowest bpm of 39 at rest. My cariologist said that was normal. Last night I went to the er cause I had the oppisite...heart was RACING all day for hours.. When I got a EKG/ECG it said the following.
Sinus Tachycardia / Possible left atrial enlargment / Right axis Deviation / RSR or QR pattern in V1 suggests right ventricular conduction delay / ABNORMAL ECG. My Vent rate was 111 bpm / PR interval was 114 MS / QRS duartion was 92 MS / QT/QTc was 318/432 MS / P-R-T Axes - 0/144/137.

I am really worried and dont know what to do.. I am a 22 year old male in otherwise good health and good weight. I smoke about a pack a day . Please help other than QUIT SMOKING

It sounds like you have a prolonged PR interval > 1.0, which is referred to as a first degree heart block. That is a delay in the electrical impulse thru the AV node.That in itself is not a big deal, but you may be going into a second degree heart block at the times your pulse drops to around 40. 39 is not a normal heart rate unless you are a very conditioned athlete like Lance Armstrong. There are a variety of conduction defects, some of which are congenital (you were born with them). There is a tachy-brady syndrome that involves both too slow and too fast heart rates episodically. With possible left atrial enlargement, I would recommend an echocardiogram and I would see a different cardiologist if he is not taking this seriously. Passing out is a serious threat to you and others (what if you were driving?) This can be treated and should be so you can feel well and function better.
