Sounds like gout to me. Its extremely painful and if he has a joint injury to that ankle the riba/intf might be aggravating it.
My husband has gout and like yours, won't do anything unless he is in extreme pain and that's how I know its the gout acting up. Don't let anybody tell you its only in the toe. My hubby gets it in knees, ankles, toes and wrist (ex football player). Its so painful he ends up on crutches during a major attack but it comes on like you described, a soreness at first, then horrific pain where he can't walk...period. It got so bad he did go to the doc and even a specialist and was put on anti inflams and allpurinal and indocine.
Have the doctor check his uric acid levels. These are elevated during an attack. Just a thought..........
