Hi - actually approx 90 % of people have a Hiatal Hernia, when the size is less than 2 inches they are not a problem. When greater than 2 inches they can cause a problem. The most usual problem is that the LES (valve) moves up a little through the gap - this then weakens the effectivness of the LES (valve) and thus reflux etc.. The other less usual problem with large Hiatal Hernia's is that part or most of the stomach will move up through the gap this will also cause reflux etc..
95 % of Gerd/reflux is not because of a Hiatal Hernia's but the weak LES (valve).
