If you are having an ob when you go into labor, you'll need a Caesarean, and so somehow you would have to explain it to her. Otherwise, unless she goes to all your dr visits too, she shouldn't have to know. You could mention to your dr that you don't want it brought up at the delivery. If he's not sensitive to your feelings, get a different dr!
I didn't have more ob's when I was pregnant with my fourth boy, but everybody's different. I was very careful about what I ate and I slept a lot. The midwives I went to cultured my vaginal area quite often when I got near my due date, and of course I glanced down there every other day or so. But I think it's best not to anticipate an ob, just to trust that everything will work out, so that you don't stress yourself; and don't pull and push your delicate tissues too much trying to see every crease or bump. That's a good way to bring on an ob!
It sounds like you have been doing something right. If you take care of yourself during your pregancy, you will be okay. Even if the worst happens and you get an ob, things will be okay, just not the way you wanted. I had my primary ob when I was 8 1/2 mos. pregnant with my third boy, and I had to have an emergency Caesarean, which was hard for me. But my boy was (and is still at 15!) beautiful and healthy.
