Herpes on men AND women lives in the nerves at the base of the spine. When the virus becomes active and comes to the surface of the skin, it can appear anywhere in the "boxer shorts" area. Does not have to be just the penis.
Even when someone is not having outbreaks, the virus can be active. It's called asymptomatic shedding. So visual alone is not 100% protection.
