High triglycerides, low HDL... appears to be numbers suited for niacin. Did you get a lipoprotein test, or a regular lipid profile? If the latter, ask for a lipoprotein test.
As for your doctor stating that vitamins not having much of an effect, that is generally true if he meant something like a multi-vitamin or antioxidants. Niacin is a well tested vitamin used for controlling cholesterol problems. Fish oil has many studies behind it too. Panthetine... eh, the studies aren't overly impressive, to be honest.
You can try dietary changes (low carbohydrate, low wheat), along with 3 or so grams daily of omega 3, and maybe ask your doctor about niacin (or Niaspan, the prescription version). They also have a prescription fish oil, but unless your copay is cheap, over the counter is a better option.
As for an angiogram, CT scan, calcium scoring, or other cardiac testing, I personally feel it's best to listen to a doctor, over any comments on an internet board. Just do your research and speak to your doctor, and it's up to you to decide. Since your stress test was abnormal, some follow-up testing is definitely warranted.
