A diuretic ususally has a profound effect in just a couple days IF you have a lot of retained sodium. If you've been on a low salt diet or freely pass salt through you (some people do!), then you won't see much effect.
Pretty basically, if your hypertension is caused by salt sensitivity (V-type,) thiazides work extrememly well. If you are a renin-angiotensin type of hypertensive, the R-type, then diuretics aren't very effective.
