Patients have the right to-

1-Be heard
2-Have tests explained in clear terms
3-Ask questions and get answers
4-Have the total attention of the Dr when in the exam room
5-Call the Dr office with concerns and get a response

Drs have the right to-

1-Know all medications (including herbal, homeopathic and suppliments) you take
2-Be told new signs and symptoms no matter how trivial they might seem
3-Know your family history, if something new comes up keep them in the loop
4-Be told if you have concerns, they cannot read minds
5-Have their own lives... if a partner see's you do not take it as a slight, even Drs get sick, have vacations or family obligations

Make it easier for you and your Dr...

Write out any signs, symptoms or concerns and take it with you to your appointment and give it to your Dr.
Also write out any medications (OTC, herbal, suppliments, etc) so that you do not blank out when filling out the forms.
Take notes when at the Dr... friends and family always ask for details, and it will help you as well if you go to other Drs
Keep a health diary, BP, HR, etc with times and dates as well as what you were doing before the readings, if the readings were done because of any symptoms and bring it with you to the Dr. Sometimes that diary itself will give the insight needed to spark an idea for your Dr leading to new treatment, tests or diagnosis.
