I keep him off sodium and he is taking about 4 meds and a dieuretic along with cholesteral meds. What next?

Uncontolled hypertension, 5 plus meds, and another health condition strongly call for seeing a hypertension specialist. And it's very likely the diuretic is contributing to his gout and his cholesterol levels as it can be a side effect.

Your husband has a complex medical history and someone having extensive experience with hypertension should be treating him. If he is already seeing a specialist, a second and third consult/opinion would be wise.
