Do I need PEP Treatment/Am I at risk?

Last night I was making out with a friend who I know is HIV positive. I was stupid and inebriated. He put lube on his penis and inserted it in me and removed it. We then had sex for only a few minutes with a condom and I briefly performed oral sex on him. Of course I woke knowing how stupid I had been (and him). I asked him his viral load and he said it has been undetectable for two years. I asked him how much he precums (pre-ejaculates) and he said very little. There was no irritation to my anus.

Should I start PEP immediately?

You are worrying needlessly.

As far as your activities during this recent episode, you need not to worry.

As you had Protected receptive or Insertive Sex and Unprotected ORAL.

your saliva contains over a dozen elements and proteins which very effectively prevent the transmission of viable HIV. I don't see any need for further concern on your parts nor any need for testing, Forget PEP.

Thank you for your response. I don't think I wrote clearly enough and just want to make sure.

He inserted his penis in me briefly WITHOUT a condom. Then he got one and we had intercourse for a few minutes. Does that change your answer? I'm worried about pre-ejaculate/precum. Thanks again.

Hi,

Sorry for missing out that bit.

Well you did have a risk but relatively speaking a lower one. HIV is not easy to transmit and it is more difficult to accomplish. And this was a single and apparently very brief incident. All of those factors work in your favor that you will likely test negative.

However, low risk is not the same as no risk. So you need to learn from this experience. You can have intercourse with anyone you want to. You need to do it the safer way which means always using a condom. No exceptions. Period.

That said, You can test at 6 weeks, 9 Weeks and finally 13 weeks. I personally expect it to be negative.

Good luck with your test from which you can get a definitive and hopefully negative result at 13 weeks after the event.

The decision to take PEP should be between you and a doctor (nobody on this forum is one). Since your partner is HIV positive and you had an unprotected exposure, it may be appropriate. PEP is considered only effective if started within 72 hours and the sooner it is started the better. Since it is a Saturday, one option might be to go an emergency room and get started on it until you can consult with a HIV expert on Monday and get their opinion. I would say that your partner's infectious disease specialist could probably give you the best advice on whether you should be on it since he/she can confirm your partner's viral load and would probably have experience advising serodiscordant couples on condom breaks and similar mishaps that result in brief unprotected penetration. There appear to be factors in your favor which mitigate your risk somewhat but they do not eliminate it and nobody here is qualified to tell you whether you should be on PEP in this particular situation. From my unqualified perspective, I can see an argument both ways.

I fully agree with Joggen. We are not in a postion to answer this question ( i have a sticky thread at the top about PEP )

I would do as Joggen says, speak to them as soon as you can if you choose to concider PEP.

Hi there,

Unfortunately you have had a high risk exposure. Unprotected anal sex with somebody who is known to be HIV positive is classed as high risk. Precum is not really the issue here, buddy as it carries very little viral load, unprotected anal sex is. It can and does induce tearing of the anal wall and the skin of the penis allowing direct blood to blood transfer.

As for PEP in my opinion, and it is only my opinion, you should discuss this with an HIV specialist at the earliest opportunity. It is only administered within 72 hours. Having said this it is not given that because you had an high risk exposure you will automatically contract HIV, but you should seek professional medical advice sooner rather than later.

Thank you to all responders. I read many other posts about this and researched my brains out. One of the things I noticed is that these situations cause people to go into a useless tail-spin without taking action or making a decision.

Ultimately, it seemed clear from the advice offered that the only thing to do was go to the ER and get a PEP starter kit and consult an HIV specialist- in collaboration with my partner's doctor- in order to gain a professional opinion about whether or not to do the full regimen. I did so. No more tail-spin. We'll see what they say over the coming days.

To those who feel at risk and are freaking out and depleting hours upon hours on google... cut it off at some point, tell an ER doctor or some other available MD what happened, and see what they say. It will save you time.

from your brief exposure you should be ok
hiv isn't as easy to get as you think.

the door still was left ajar, so the path
you are taking, is the best one to travel in your case.
you will be ok

I was also the receptive partner of a broken condom exposure on 4th, and I've been on PEP since the 5th. How are you faring with PEP? Just thought talking to someone else in my situation will make this all a little easier. Hope to hear back..
