My physical therapist did a test and said I have BPPV. When in a certain position, I had vertigo and nystagmus for about 15 seconds.
Here's my question... she performed a maneuver, during which the same position provoked the vertigo. After previous attempted maneuvers (we've been working on this for about a week), I felt extreme vertigo about 10 seconds after the maneuver was over. This time, though, I had no vertigo at the end. I thought this was a good sign. However, about 45 minutes later, I was standing at home in my kitchen and had an episode of extreme vertigo. Because my PT said to limit my head movement for the next couple of days, I was being extremely careful and know that I didn't put my head into a position that typically triggers the vertigo. The vertigo came without a significant head movement.
Is this normal or a sign that the maneuver probably didn't work?
As someone who has experienced close to 20 BPPV events in the past couple of years and been through many Epley Maneuvers .... I will tell you that I have had similar experiences.
From my experience, if I get vertigo when I sit up at the end of the maneuver - it has been an indication that the crystals have not been repositioned correctly - so I'd recommend doing another round of the maneuver at that point. Also - I have had the experience of the extreme vertigo within hours after an Epley. My vestibular therapist explained that that can happen if the crystals were not repositioned correctly or they got stuck in an area and after a while they release and cause the vertigo sensation. Again, that means you need to do another Epley to get the crystals repositioned.
I would take a look at the "Sticky" posting (top of the list of posts here) that talks about the self Epley. You can easily do this maneuver at home - especially now that you've been through it and know what to expect.

Thanks for your reply! My PT taught me a home maneuver and, after deciding that the earlier maneuver didn't work, I did it last night. No vertigo immediately following and none in the hours after. I'm optimistic!
Another issue, though, is that my first and primary symptom has actually been balance issues and non-vertigo dizziness. The vertigo actually started about two months into experience these other symptoms. Unfortunately, I do still have some of the other symptoms today, but at least I haven't been hit with the vertigo yet.
Can you tell me how often is the epley recommended for bppv? do you do it once, wait a week, and then see if you still have symptoms, do you do it several times a day until symptoms subside? Are you supposed to do several reps at a time? What did your ENT suggest to you? Are there any weblinks that you know of which provides information regarding how often you should do the epley once you exhibit bppv symptoms? i was diagnosed 5 years ago. recently had a reoccurance. i am SO SCARED of doing the epley, which forces myself to be dizzy. i did it once a week ago, but i panicked and couldn't hold the correct position long enough. planning to try again soon (although hoping it would've went away on its own). I just need to know what i'm expected to do, b/c i'm tired of these episodes, and would like it to go away. this is my 3rd reoccurance.
