Very strange and troubling

I keep getting the sensation of blood/liquid 'gushing' inside my ear, it's kind of like when you put a shell to your ear but it happens when I am off vertical, working out, shouting. I also have a constant ringing - I'm assuming to be tennitus - but over the top of that I get a louder, higher pitched ringing sometimes, like a whistle. I went to the doctor about it and he said I have an ear infection and gave me some drops for it but that just made the situation worse. Unfortunately I was "out of the catchment zone" for that same doctors so I had to join another and he looked and said he can't see anything wrong. Any help regarding this would be appreciated, thank you!
