Question for fellow IBSers and anyone who can help

HI everyone---
I've suffered w/ IBS for a number of years. I had a colonoscopy done recently because my symptoms are so much like an inflammatory bowel disorder, no inflammation was seen. They removed one polyp and did not do a biopsy. I am in process of a positive diagnosis for Lupus and am in flare currently. I took 40 mg prednisone for 7 days and 80 mg prednisone for 7 days and low and behold ----I WAS REGULAR!!! I HAD NORMAL, PRODUCTIVE REGULAR BM'S WITHOUT PAIN. This hasn't happened at least since I married my husband four years ago, he has never known this to happen for me. I am the chronically constipated kind of IBSer who is constipated no matter what. I also have bad bowel flare up problems and then have diarrhea as well. My question is... if prednisone made me regular, do I need to look further into an inflammatory condition like ulcerative colitis? Would IBS respond to prednisone like this? As soon as I went off the prednisone, I didn't poop for 7 days and now my bad bowel flare problems are in full force. Also wondering about the biopsy thing, is there inflammation that can occur that can't be seen or that doesn't occur all the time? Does anybody know any of these answers??? Hello Shawnee,

I don't know if I have all the answers you asked about, but I have suffered from IBS for many years as well, have a GI specialist and have asked him many questions. Typically prednisone is not prescribed for IBS since it works to reduce inflammation that is not usually present with IBS. If your colonoscopy did not show inflammation, there still could be an issue with what is called microscopic colitis. This requires a biopsy to positively diagnose. There are other forms of IBD that also require a biopsy to properly confirm. You mentioned that you are normally constipated but have bowel flares. How often do these occur and do you have severe diarrhea and/or notice blood in your stools? Just to be certain, I would definitely discuss with a GI specialist what further testing might be warranted, if nothing else just to set your mind at ease. Sometimes with Crohn's disease there is inflammation in the small intestine which would not be seen during a colonoscopy.

You have been very helpful!! I have felt concerned after I noticed such a dramatic improvement after prednisone use and such a dramatic worsening in my constipation after it stopped. I tested positive 3/3 samples on a fecal occult test which is why I continued with a colonoscopy. They removed one polyp and my GP said that was probably why I was bleeding however I have continued to have blood in my stool, on my toilet paper, underwear, and just blood in the toilet so I don't know how confident I feel that my bleeding has been taken care of. I am constipated always despite fiber therapy, exercise, water, etc... you name it I've tried it and it doesn't help. I had no improvement whatsoever on Zelnorm either. My bowel flares have become worse every time I have them, my last one however was better than usual as I started using Bentyl at the first stomach cramping (which is kind of the beginning of my flare activity). Before this most recent one, my last was in May. After I had my second baby 18 months ago, they were occuring about every three weeks. I have joint pain, muscle aches, mouth, nose sores, and a host of other symptoms and have been diagnosed with lupus by my GP and am waiting for an appt with a rheumatologist. I'll ask for this rheumatologist's opinion and if they can't give me any answers, I think I'm definetely going to get an appointment with a GI specialist even if it just puts my mind at ease. If you have Microcytic Colits you would respond to the steriods. When my colitis flares up I only respond to a cortisone type medication, I forget what the name of it is now as it's been a few months since I was on treatment.

Did they do a biopsy for Microcytic do you know? Chances are if they did a colonoscopy they would have but I would ask to confirm it. It also corrected my bowel movements. Prednisone on the other hand will cause diahrhea which may be why your bowel movements were normal since you said you are always constipated.

It is also highly possible you do have Chrons as indicated, it will product joint pain and of course your Lupus will as well. I am assuming you had the bloodwork completed to get the positive result. I'd check with the rhuemy to get his/her thoughts on it and go from there.

THanks very much for your input Kissa! I am definetely going to discuss this concern with my rheumatologist and if my problems continue I believe I'll make an appt with a GI specialist. I had a colonoscopy but no biopsy was taken, just a polyp removed. This was supposedly the reason for the bleeding but I have just as much bleeding now as ever. I did not know that PRednisone was a diuretic either and that could possibly be why I became regular, it was very welcome! Thanks again for your help!
