You don't say how bad you diarrhea and pain are, but those are the main symptoms I have and have been diagnosed with IBS. My pain is bad enough I am on daily pain meds for it. However, there are several other things that it could be too. There is a condition called Clostridium difficile (or C. diff for short). I had that last year on top of my IBS. The first things a GI doctor would ask you, is have you spent any time on antibiotics? Have you been or do you work in a hospital? Those are the main ways you contract C. diff. There's also Colitis. It's amazing how many people have one of the various forms of colitis. There are several other things too, such as Gastritis.
Your best bet would be to see a GI doctor, if your appointment you have isn't with one. When I first began having problems, my PCP referred me to a GI doctor who ran many tests, the first being a colonoscopy. If you haven't had one, you should. The second test should be an endoscopy. These two tests can many times shed some light on your problem. Good luck.
