Sorry I haven't experienced your situation, but I have a comment about the nicotine. If you'd rather not get it by smoking, I wonder if one of those nicotine chewing gums, used for smoking cessation, would do the trick? There are patches as well, but maybe you get more of a quick hit from the gum? Really out of my field here ... just wondering.
A lot of people rely on caffeine to get things moving. Have you given that a try?
