| CKD OR AKD ... Confused about my moms tests
Hello,
My mom suddenly fall sick. Had fever for 2 days she dint had any food or water for this two days. No dibates no hypertension. After doing the blood test, got the reports at night, found her creatinine level at 18.2, potassium at 7.. Next day when we visited a nephrologist he did the all blood test, electrolysis, hiv, hba1c and other test.. The creatinine was at 13.2 and potassium was 7, wbc where high as 21600. He told to do a dylasis as the levels were high and she was not well. After dylasis he again did the crea an potassium test and told us to wait and see how well the kidney recovers and started antibiotics. After a week the crea reached 7.60 and potassium was 3.40, wbc where still high 22400. He told us that now we need to do a regular dylasis or a transplant. The kidneys arent working well. We refused to do and told him to wait for 2days. She was still on antibiotics and calcium tablets The crea gradually got lower slowly but potassium got increased to 6, then he gave few medications to lower the potassium, the electrolysis also got disbalanced... Sodium n protien got lowered.. After 2 weeks the crea is at 4.05 and potassium is 3.80
Protien: 5.20
Almubin:2.70
Sodium: 121
Hoping to get the creatinine to get lower
But i am concern abt what exactly is the situation with my mom..? Is it a acute kidney disease or chronic kidney disease. As the hba1c shows 4.8% DCCT. The normal range is 4-5.7. This confirms she was not dibatiec. So whay exactly is the causd of this condition
Please help me.. Any other test to perform or any questions to ask the doctor?

I am not a doctor but it sounds to me as though she has some sort of infection that has damaged her kidneys. Even at a creatinine level or 4.8, many people are already doing dialysis at that point. The doctors cannot read the future. It is possible, that with some additional dialysis treatments her kidneys may recover more. This will be seen through additional blood work (easy to do when she is on the machine). But it is also possible that the damage is permanent.
I'm very sorry to say that only time will tell. In the meantime, be aware of her levels of potassium. Too high (well over 5) and it could cause her heart to stop. So you need to listen carefully to doctors instructions regarding diet and fluid intake.

I was concern about, does any kind of infection causes permanent damage to kidney..? Damage leading to kidney failure..?
Done the test today..
Wbc is lowered to 10400
Creatinine: 4.15
Potassium: 3.9
Anyway my mom's health with creatinine 4.15 is good.. I mean no loss of appetite. She takes proper diet and also urination is good. She feels a little bit drowsiness and tired.
When is dylasis required normally..?
Is it just when the creatinine level is high and no other symptoms or creatinine level with some symptoms needs a dylasis
Does she requires a dylasis now..?

When to start dialysis can be kind of complex. It is a decision made between the patient (and their family) and the doctor. Here are some of the factors involved:
1. fluid levels - if your Mom's kidneys can no longer take off enough fluid and she begins to not only get swelling, but also has fluid resting around the lungs and heart (this can be dangerous)
2. potassium levels - as I said before, high potassium can be dangerous, so once it's been very consistently too high and is no longer affected by restricting the diet, dialysis becomes necessary
3. illness - how badly she feels. Some feel terrible at a creatinine around 4, and others hold out much longer (I was at about 10, but should have started earlier - and I was weird) For this you will have to keep an eye on her as well. We the patients often don't realize how bad we feel because it snuck up on us - but families see how terrible we look
4. extreme fatigue and mental fog - again, you as the family will have to help monitor this
And yes, some infections can lead to permanent kidney damage as can drugs like Ibuprofen (don't take any now!).

Did the ultrasonography.. It shows Both kidneys are normal in size, shape and position. The parenchyma shows increase echo-texture. Corticomedullary differentiation is well maintained. No evidence of calculus or hydronephrosis seen.
Right kidney measures 9.9x4.1 cm
Left kidney measures 9.2x3.5 cm
IMPRESSION :
Bilateral medical renal disease
