Nail changes can be a sign of many diseases, but one of the most common/important nail changes in chronic disease is clubbing, and that is not related to kidney disease. It is related to conditions which result in scarring, cancer or purlent infections of the heart, lungs or GI tract.
heart - subacute bacterial endocarditis
lung - cryptogenic fibrosing alveolitis, lung abscess, lung cancer
liver - liver abscess, liver cancer, cirrhosis
