creatinine 3.4?

sir,
My father (age 53) is a diabetic patient from last 12 years and also a BP patient . every thing was normal but in last few month he have been suffering from kidney disorder .after consulting a doctor and going through few test his profile is as below.

1. creatinine 3.4
2. hypertention
3. type 2 DM
4. CKD (stage iii )
5. weight 70 kg
6. Height 172.5 cm
7. BMI 24kg per meter square
8. BP 140/80 mm of hg
9. CBS 248 mg/dl
10. Diabetes pp (ie after meal) 320

His doctor has prescribed him the following medicines
1. Tab minipress - XL (5MG)
2.Tab amlogard (10MG)
3. Tab pantodac (40MG)
4 Tab aztor (5MG)
5 Tab shelcal CT
6 INJ . human.Mixtard 30/70
7 Tab ketosteril
8 SMBG
9 Tazolac
So kindly help me out whether the medicine prescribed by the doctor are sufficient or not and what does the creatinine 3.4 indicate?is there any serious problem ?

I'm sorry, but I don't recognize any of the names of the drugs. Could you tell me what they are supposed to do, or find another name perhaps more common in north America? I suspect that there is probably 1 or more bp meds (one is probably an ACE inhibitor or an ARB as they are shown to protect kidneys). The insulin is very important as his blood glucose is too high. It should be less than 140 (or 180) 2 hours after a meal and less than 100 fasting. This will also help protect the kidneys. The bp should also be a bit lower to help the kidneys. Is he eating a low sodium diet? This will help preserve kidney function as well.
