Complex Meniscus Tear with meniscal Cyst

52 y.o. male, have ahd problems for 6 months with left knee pain. No trauma. I had an MRI which revealed the following: complex tear of the posterior horn of medial meniscus which extends near the posterior root, with a primarily horizontal component. An intrameniscal cyst may be present. Mild extrusion of the meniscus by 4-5 mm, resulting in bowing of the medial collateral ligament. Ther is mild cartilage irregularity in this compartment. The medial collateral ligament is intact. There is small amount of fluid in the deep infrapatellar bursa. Very small joint effusion. Small amount of T2 hyperintensity anterior to the patella. Eccentrically located sclerotic focus within the proximal tibia shaft medially. I have an appointment with an Orthopedic Surgeon next week.

Curious if anyone knows if the only real issue in the above report is the meniscus tear (which i assume needs surgery).
Are the other issues, conditions that don't need to be addressed with surgery?
Any in sight is appreciated.

My recent (8 wk ago) meniscus tear didn't need surgery. Happened on a Monday (no idea how). Swelling was bad. Saw knee doc Thursday and he withdrew fluid and sent me for an MRI. MRI showed small meniscus tear and arthritis (which I figured). Doc did an injection of cortisone and told me to do some quad exercises to strengthen the muscle and light walking or recumbent stationery bike. Cortisone worked well (though I had sort of a bad reaction). If there is no more swelling or pain he won't operate on the meniscus. I really thought he was crazy but here I am nearly 8 wk later with no pain (minimal moments of pain but can do light walking and no swelling)
