Chronic Cough (with other symptoms)

Hi all--I hope that someone can help or at least provide some reassurance. I will start by stating that I am seeing a pulmonologist next week and hope to get this resolved. I just want some ideas here and maybe symptoms I should tell him that I haven't tried.
I have had a cough since March, BUT it has changed some. In March it was just an allergen-type cough and just came every now and then. It has become chronic since about May 25 or so, and it has gotten worse in the last couple of weeks. It started after a long drive, then foolishly laying in a hotel room for a few days doing some computer work (rather than sitting). That is when it picked up.
I also should mention I went to a chiropractor at one point due to back pain from the drive; the cough got significantly worse after that.
It is a mostly dry cough, although sometimes I feel like I am coughing up mucus. The cough is often coupled by an inability to catch my breath. The most consistent pattern I can see with it is the one that the three doctors I've talked to just haven't paid attention to, but it is definitely not a spurious correlation: It is significantly worse when sitting or laying on my back.
To elaborate a bit more on that, when I am sitting with my back pressed against something, the cough is more consistent. When I am laying on my back, as opposed to my side, it is more consistent. When I press on my rib cage on the middle part on my right side, I can feel pressure and cough from that. I also have pain under my shoulder blades. I have slight pain in my chest.
Occasionally I have what feels almost like bubbles coming up through my heart.
I do trail running (sometimes). Last week I hiked/jogged over 6 miles on a hilly, rocky trail. I did not get short of breath once. I did not cough. I got into my car and sat and felt pressure then the coughing came.
Last week, I had a sharp pain in my mid-right part of my back while sitting. As that pain increased, so did my back. My doctor told me the two were completely unrelated. I am sitting as I type and feel the pressure.
Other things: I am hoarse, but I have also been coughing a lot.
I was treated for Hodgkin's disease 12 years ago and received chemo and radiation; obviously secondary tumors are a big concern of mine, but that doesn't appear to be the case (that I know of). It is my biggest worry right now, but other tests just don't point to that.
I was also in different locations (three different states), so I don't think it is allergies in that sense. I was wondering about mold or something in my A.C. but it is just as bad in my car, my office, and my house.
Tests they have done:
Blood work: platelets on low side of normal, but still in normal range. TSH on high side of normal.
x-ray: some fibrosis that hasn't changed since last year or the year prior (scar tissue from radiation)
no wheezing; no swollen lymph-nodes (a bit sore)
oxygen is fine; blood pressure fine
Medicines we have tried:
inhaler (made it worse)
Flonase (not sure if that is helping)
doxycycline (sp)--still on it; have been on for about a week
medicine for acid reflux
