I am at a loss...

Thank you in advance for taking the time to read my thread. To make a long story short, I will try and break it down for you. I am at a loss...

I am:
*38-year-old female (will be 39 next week)
*5' 8"
*137 lbs
*1 child (11-years-old)
*No miscarriages

I have a family history of:

*Mom: RA; Osteoporosis; osteoarthritis
*Father: Renal cell carcinoma - kidney; prostate cancer; melanoma
*Sibling: Hodgkin’s Lymphoma

I am experiencing the following symptoms for 10+ years that is progressively getting worse:

*Shortness of breath
*Stiff, achy all over
*Ache in my chest - sometimes "stinging"; sometimes a "pulling" sensation. Both located "deep" in my chest.
*Lower back ache...throbbing
*Tingling sensation in my arms, hands, legs and feet
*Hands get red and warm for no reason
*Memory loss

I am on the following meds:

*Benicar - 10+ years (High blood pressure)
*Cymbalta - 3+ years (Depression)

Following tests have been completed:

*ANA Nuclear Titer - Positive
*D Dimer - Positive
*Aldolase (muscle enzyme) - Normal
*CCP - Negative
*THYROID PEROXIDASE AB - Normal
*Vitamin D - Normal
*RHEUMATOID FACTOR, QUANT, <20
*CT Chest - Normal (checked for blood clots due to positive D Dimer)
*X Ray L-Spine - Bones appear mildly osteopenic but intact

I thought perhaps this could be fibromyalgia, but I have no "trigger point" soreness or pain. Then I was wondering if this could be Lupus, but I have no inflammation in my blood work. I am depressed over this as I know I feel pain. Any help, suggestions and/or comments will be greatly appreciated!!!!

Mimi, hi & welcome. I'm sorry you're struggling.

Re: ANA, a big problem is that it can be positive in a variety of conditions, including lupus, rheumatoid arthritis, and other rheumatological conditions; also quite a few outside of rheumatology; or simply due to a passing virus, or aging, or certain prescription meds.

Once your ANA came back positive, were *further* antibody and immune determinations performed, in addition to rheumatoid factor (which you mentioned)? These include things like anti-ds-DNA; C3, C4 and CH50 serum complements; anti-Ro; anti-La; anti-Sm; anti-RNP; and antiphospholipid antibodies. BTW, rheumatoid factor, interestingly, can be positive in either RA or lupus... Did your dr. say what RF <20 means?

Also, quite a few drugs have been implicated in a subtype of lupus called drug-induced lupus erythematosus (DILE), including blood pressure meds. I think anti-ss-DNA is the autoantibody most associated with DILE.

There are "sticky posts" with permanent info at the top of the thread list with info on diagnostic criteria, symptoms, tests, etc. Have you seen those yet?

Maybe biggest question is, have you seen a rheumatologist? All these tests are tricky & best done by a rheumatologist, who is also the specialist best at reviewing your lifetime medical history for clues & signs.

I don't think elevated ESR is "required" in lupus---common, probably, but not required in the sense of being a diagnostic criteria. If I were in your shoes, I'd want a rheumatologist to consider the possibility of systemic lupus or drug-induced lupus & run more tests. I hope this helps some. Post more when you can, OK?
