It is possible to have lymphoma without enlarged lymph nodes, although this is rare. Some lymphomas can start in non-lymphoid tissue, although usually when it is found in other organs it is the result of metastasis from another site. There is also a type of lymphoma that starts in the spleen.
It should be remembered that the spleen is part of the lymph system, and that for staging purposes - at least according to the Ann Arbor scale - it is treated as just another lymph node (in other words, lymphoma that is found in the spleen is not considered extra-nodal).
But because the spleen is part of the lymphatic system, it can also swell due to infection (the Epstein-Barr virus is one condition that can cause this). A swollen spleen is a common clinical finding in people with both acute and chronic diseases, and in some cases it has been observed in otherwise healthy individuals.
