What can cause submandibular lymph node swelling?

Hi everyone,

I'm a 27 year old female looking for a bit of advice. About 10 months ago I went to my doctor because I was feeling a bit run down and I couldn't seem to shake it off. When she examined me she found my right submandibular lymph node was swollen (0.5cm) so she ran some blood tests. The tests came back as a low white blood cell count and anemia with a hemoglobin count of 9.9. Because this persisted for a while she sent me to see a hematologist. By the time I saw one 2 months late the lymph node was 1.5 and I also had swollen nodes in my left armpit and posterior triangle (all between 1cm and 1.5cm). As it persisted until March with no improvement in my Anemia or low white blood cell count I was referred to see a ENT surgeon for a possible biopsy. However the hospital that I visit is a teaching hospital so I am under the care of one senior hematologist but the decisions are made by students and I see a different one each time. By April one of the students was convinced that the lymph nodes were smaller than what his colleagues had estimated so cancelled the biopsy and they are still trying to figure out why I am anemic. The problem is my right submandibular lymph node is continuing to get larger. I went back to my primary care doctor who is not happy with the treatment I have received as he said the submandibular lymph node is now at least 4cm and it resembles a large egg sticking out of my jaw and he is not happy with the specialists and the fact that they lymph node hasn't even been scanned. I've also had drenching night sweats - I have to get up in the middle of the night to change everything and dry my hair and by the time I put my sheets in the wash the next day they are coated in white sweat marks. I've also had intense itching which I have been complaining about for months and the only way to describe it is that it's in the muscles of my legs and the only semi relief I get is from walking around and shaking them.

So in a nutshell my primary care doctor has ordered an ultrasound and chest x-ray as the first step and said we will see where we go next once he gets those results. I don't expect answers or anything like that but naturally as this has been going on for quite some time I am beginning to get a little concerned especially as my doctor stressed the importance of getting to the bottom of it as soon as possible. Is submandibular lymph node swelling usually a symptom of lymphoma and are there any other reasons for it (It generally isn't painful only slightly uncomfortable just before it gets larger, it's hardish), any input or thoughts would be appreciated! Also both my doctor and dentist have given my teeth, gums and other glands inside my mouth a clean bill of health. I haven't had any chest or respiratory infections or other viruses and things like autoimmune conditions, viral infections, TB etc have all been ruled out. Infact apart from my low white blood cell count (lymphocytes and neutrophils) and anemia everything else is within range.

Re: What can cause submandibular lymph node swelling?

Have just been tested for thyroid nodules and the ultrasound has found 'lumps' in my submandibular glands. Also have a very swollen left neck gland and the ultrasound showed a cyst (maybe the two are connected).

Have been really ill for 10/11 weeks - extreme fatigue, chronic sore throat into ears, ringing ears, dry mouth and hoarseness. Was given antibiotics which has relieved the sore throat and fatigue - hoarseness has improved but still there as is dry mouth and vague sore throat and ears.

Doctor said that thinks that this has been going on for a while but did not have obvious symptoms (was rather stressed so did not ask any questions).

Waiting for the surgeons letter as will have to see two different specialists .

Just wondered what the outcome for you has been as it does not seem like a common problem.

Hope all is well and on the road to recovery.

Re: What can cause submandibular lymph node swelling?

Hey, I'm also curious as to what happened. I have had a swollen submandibular lymph node for slightly over a month. It has no pain, but it is very large and moves from under my jaw to above. I've had an ultrasound to confirm that it was a lymph node. I am waiting for the results of a white blood cell count and also my doctor wanted to check my thyroid due to complaints of extreme constipation (never had the issue before). Lately I've noticed some ear pain and headaches. My doctor has put in a referral to an ENT for biopsy. Concerned and eager to talk to someone who's experienced something similar.

Re: What can cause submandibular lymph node swelling?

I have also had sever sore throats and ear ache for almost 3 months. Very fatigued no energy. Had antibiotics and this helped the throat situation but then had blocked nose, loads of mucus, coughing. Ringing ears, fuzzy head. Very hot then can be cold but have days of hot flashes and can break out in a sweat at night sleep with window open and just a sheet but on some nights get really hot and clammy. Oh yes, have a cyst in my left neck gland that has got really big and hard but my lumps in the sub glands where picked up on ultrasound that had as they thought I had a thyroid problem.

Am off for bloods today and only have an appointment with the ENT in September (that is our health system). If there is something there am hoping that the bloods will pick it up and then my appointment will be sooner rather than later.

Have good days and think everything is clearing and then can wake up and just feeling unwell with odd symptoms.

Please keep in contact as it will be interesting to see what becomes of both our health issues and perhaps helpful for others in the future as there is not much out there relating to this situation.

Let me know your blood results and what the doc has to say about them.

Sending butterflies of health your way.

Re: What can cause submandibular lymph node swelling?

Hi there

Did/do you have lymphoma? If so would you feel like you where getting better and then suddenly get ill again, especially with extreme fatigue. Oh yes forgot to mention that part of my symptoms have been a really dry mouth and a hoarse voice for almost 3 months (this can be better on some days compared to others but my voice has def changed.

Just had some bloods today to test for a virus and for lymphoma (prob white cells etc as they cannot tell from bloods from what I have read).

Thanks for any feedback.

Re: What can cause submandibular lymph node swelling?

Forgot to say that my main symptom has been hoarseness - some days hardly able to speak and others a bit better. The last few weeks broke out with 2x huge big bruises on my arm - probably the size of a big plum.

Had the bloods this afternoon for viral and lymphoma (white cells etc), will have results by end of week.

Re: What can cause submandibular lymph node swelling?

Hi there no I dont have lymphoma, but they panicked when i had some of these things you have. In Australia, we have a good medical system I suppose and I was able to go straight to a specialist. He first did a gastrophy, a colonoscopy, bone marrow biopsys, which scared me half to death, but they knocked me out and didnt feel a thing. Next to really look for lymphoma after all that they did a liver biopsy, that was the worst thing I have ever had. Then a cat scan with dye, xrays of chest, ultra sounds of abdomen, I eventually went to my sons rhuemotologist, and he did more extensive blood tests and found all the counts with hb and wbc, was related to the sjogrens. But I can now say that i have a everything tested. BUT if you have sjogrens, you have a higher chance of developing lymphoma, which made me feel awful, but I am fairly healthy, so these are all the tests you should be having...asap....One thing I learned during my health crisis, is I appreciate life so much more, hope you have more luck getting this resolved. Its not right to just let time go by without a definite diagnosis.

Re: What can cause submandibular lymph node swelling?

My bloods came back showing nothing unusual. After 2months 3 weeks my sore throat, hoarse voice etc have cleared overnight. Will just have to say must have been viral.

Re: What can cause submandibular lymph node swelling?

Glad youre doing better kaz. My cbc looked normal, thyroid was good. Waiting for an ENT to call about my referral. Started antibiotics but thats not doing anything for me. My ears feel worse lately. And I think I have a new lymph node under my breast that is big and like a marble. UGH I want to feel well. Still constipated too. Didnt go for almost five days. I hate all this waiting.

Re: What can cause submandibular lymph node swelling?

The waiting is the worst. Have you been unwell for a while and what are your symptoms?

I still have to see the ENT as the ultra sound showed these lumps in the sub glands and a cyst in my neck which seems to go up and down at different times, but am soooo much better and have my energy back after such a long time. Will just get everything checked in the next 6 weeks for peace of mind, but am sure all is fine.

Hope your referral is soon and keep in touch with the outcome.

Re: What can cause submandibular lymph node swelling?

I seem to have the same watch and wait - perhaps they wait to see if it passes like kidney stones. All my symptoms gone although every now and again wake up at night with very sore throat and ears which goes by the morning. Still have a hard swollen neck gland on left - but very well compared to 3 months of thinking that had a serious problem.
