(Sorry for the long post)
I have read GeoffB sticky and other forum posts but there are few questions that have never been answered, related to actual application of topical creams. Its almost 20 days now I have been daily retracting and washing under the shower as suggested by the doctor. To my good luck, I am able to pull the foreskin beyond the head and wash properly, although I find the tissue too soft.
1) Geoff suggests putting least amount of Betamethasone and only to the frenular band area and no where else. My doctor suggested me to put 1% Hydrocortisone, never prescribed betamethasone though. He told me to retract the foreskin as much as possible and then apply on the exposed skin. Reading into the mechanism of how this cream works, is there any problem if the cream gets onto the glans penis? Every time I apply this method, the cream invariably gets deposited on the glans penis. since I can pull back beyond head, when I apply a tiny bit to the rim of the exposed skin, it gets deposited to the glans, near to the back of the head. I have tried gently skin massaging but the cream does not spread.
So what is the best way to apply cream.
2) Even though I able to pull back on flaccid state, I can see that the tip (frenular band) is pretty tight and kind of walks over the glans during retraction like squeezing with an elastic band. How do I keep most of the cream onto this small ring that gets exposed only when pulled back? I can see that the rest of skin is quite elastic but this band is fairly rigid. I have to infection or anything as per my doctor.
3) How many times a day should these creams be applied and does the head be cleaned everytime with warm water or something? Also, should the stretching be done immediately after the application or wait for it to act and do streching like an hour later
4 ) Are there any side effects of this cream if applied generously or if it gets on the glans penis?
5) Is it okay to apply oil when retracted under shower, so that further retractions become easier? I applied almond oil for two days and it seems to help. (I like almonds as they are all-rounders)
6) Should I ask my doctor to prescribe Betamethsone or do I just increase the amount of hydrocortisone application?
I would really appreciate for any response from experienced users. there seems to quite a few like Geoff, Tommy and others.
Re: Help with Phimosis cure and topical application.
The steroid cream should be applied to the outside of the frenar band.
It shouldn't come into contact with the head. The steroid cream thins the skin.
You should talk with your doctor regarding how to apply it. I have no idea how the cream would work with a tight frenulum. Good luck, it sounds like you've made considerable progress.

Well Tommy, something that I can't understand is that how it is possible to apply the cream without it being gettin in touch with the head. The problem is that once I stretch the foreskin back only enough to reveal the frenular band which is now about probaby 1.5 cm in diameter, and put the cream on the outside, let the skin roll back over, the cream invariably touches the head. So it seems one has to hold the skin back and apply cream and hold it there using a gauze or something.
Regarding doctor, I gave them a call and the nurse (who talked to the doctor) said there is no way to avoid it and it is okay to apply generously. When I told them that it seems I get a little numbness in the head after I apply cream, and my tip (just near the opening) has become very sensitive as if it has been bruised or swollen. He kind of then backed off and said to stop the cream and get a second opinion somewhere else. Great! Now I am back to square one to get another appointment elsewhere.
Re: Help with Phimosis cure and topical application.
the cream is supposed to go on the outside of the foreskin, when the foreskin is not retracted. this doctor doesn't seem to know a lot of what he should know.
if it were me, I'd see someone else, ask for betamethasone cream, and get really clear instructions on how to apply it. good luck.

It will be another few months before I will get an appointment with a urologist here. Its too busy time I guess.
So since these instructions aren't given clear on the sticky also, may I re-iterate the instructions as you know it so that at least I can start with hydrocortisone itself.
"Without completely retracting the whole foreskin, gently pull the foreskin only enough to reveal the frenular band and apply the cream on the outside very little." Is that the right instruction?"
A question: When should one optimally do the stretching exercise. Like apply the cream, wait for sometime and then do it for few minutes or as long as you could without hurting. Or you should stretch and then apply the cream. I know showering helps a lot as it sort of loosens the skin a bit. Any proper instructions here.
Tommy, thanks a lot for the advice. I really appreciate it.
