The fact that it is painful is actually a good sign. It suggests that it is a benign epididymal cyst. This can easily be verified with a 2-minute ultrasound examination. The operation to remove it is usually an oupatient job, although it will require general anaesthesia. If you are able to go direct to a urologist, that is the best plan. Otherwise get a referral.
